Booking Form

1. Acknowledgment of Risks

I understand that participating in a chartered fishing excursion involves inherent risks, including but not limited to:

  • Slippery surfaces, sudden boat movement, and trip hazards
  • Equipment malfunction or misuse (e.g., rods, reels, hooks)
  • Changing weather, tides, and sea conditions
  • Encounters with marine wildlife (e.g., stingrays, sharks)
  • Risk of drowning, sun exposure, or motion sickness

I acknowledge and accept these risks.

2. Release of Liability

I release and hold harmless Chasing The One Charters, its owners, captains, crew, and affiliates from any liability for personal injury, property damage, or wrongful death resulting from participation in this excursion, whether caused by negligence or otherwise.

I agree to this release of liability.

3. Medical Fitness and Emergency Care

I certify that I am physically fit to participate and have disclosed any relevant medical conditions.
I authorize the crew to seek emergency medical treatment on my behalf if necessary.

4. Safety Compliance

I agree to follow all safety instructions provided by the captain and crew, including wearing life jackets when required.

5. Alcohol and Substance Use

I understand that alcohol may be consumed onboard at my own risk. I agree to behave responsibly and will not operate any equipment while impaired.

6. Photo and Media Release (Optional)

I grant permission for Gulfstream Inshore Charters to use photos or videos taken during the trip for promotional purposes.
I decline photo/media release.

7. Minors

I am signing on behalf of a minor and accept full responsibility for their participation and safety.

8. Cancellation and Weather Policy

I understand that trips may be rescheduled or canceled due to unsafe weather or sea conditions. Refunds or credits will be issued per the company’s cancellation policy.

I acknowledge the cancellation and weather policy.

9. Governing Law

This agreement shall be governed by the laws of the State of Florida. Any disputes shall be resolved in Sarasota County.
